If you need to order supplies or recently changed the needle you’re using, please use the form below. All items are provided to aluetta patients free of charge.

- Call the Momentum Patient Support Program at 1-877-724-9361. We will send you a shipping box and prepaid waybill for your device.
- You must remove used needles and cartridges from the pen.
- Ship your device to our approved recycling partner.
